Changing the name on your account

A change to the name on your account can be accomodated, typically in specified cases, by submitting a request form and supporting documentation.

Note that name changes are Not Permitted on accounts where a new person will be taking over the account except in cases of death or power of attorney.

Required documentation

First you will need to provide one of the three supporting documents listed.

Then, download the request form linked below.
Fill it out, and mail it along with your supporting document to complete a name change on your account.

  • Marriage/divorce certificate
  • Legal name change documents
  • Power of attorney certificate

Name Change Request Form

RCN permits the name on an account to be changed in the event of marriage, legal name change, divorce, death, or power of attorney.

Please complete this form and return to RCN. Requests may take up to 14 business days in order to be processed. The request will be denied if the required documentation does not accompany this form. Submitting false documentation or misrepresenting your identity may result in legal action in which case you may be penalized to the fullest extent of the law.
